Job Description This position provides direct clinical services to clients in the FMHC/FFACT program with at least 75% of services occurring in the community. Provides counseling, case management and advocacy to FFACT clients, with the goal of reducing recidivism into the Jail, as well as when appropriate, working to reduce reliance on ER's and use of both medical and psychiatric inpatient hospitals. Assists clients with developing and using natural supports, with obtaining and maintaining stable living situations, obtaining necessary medical care and with the development of healthy and positive coping skills. Attends court with clients on a weekly basis and as necessary responds immediately to crisis situations with FFACT clients. Responsible for rotating on-call shifts as assigned. The Substance Use Disorder Professional (SUDP) serves as the primary contact with clients, is responsible for screenings, assessments, development of treatment plans and providing individual, group and family counseling. Provides program information and facilitates referrals to outside resources to facilitate recovery and ensure client compliance with all legal requirements.
ResponsibilitiesProvides effective clinical case management services to improve FFACT clients' ability to live successfully in the community.Performs on-going clinically appropriate assessments of clients. Develops, with the client's voice, the goals to be addressed in individual treatment plans in accordance with the appropriate model of care that are recovery-focused, person-centered, meet Medicaid standards, and are measurable. Provides behavioral counseling, case management, and crisis and stabilization services to FFACT clients, primarily to client's on SUDP's caseload but potentially to any clients being served by FFACT. Coordinates care with internal and external mental health providers. Maintains clinical competency and resource knowledge required to meet changing client, community, and industry trends. Completes written reports for the court and attend scheduled court review hearings with clients on a weekly basis. Provides advocacy with the court system, the Jail and other community supports as needed. Attends treatment planning meetings and daily team meetings. Maintains professional clinical boundaries with clients.
Provides direct SUD service to FFACT program. Assures compliance with agency/program policies and procedures, all WACs, and contractual requirements. Maintains compliance of 42CFR regarding confidentiality of client information. Complete SUD Assessments for all FMHC clients, at start of program and as needed. Make appropriate referrals for treatment based on assessed level of care and client's benefits. Serve as team point of contact for all SUD treatment providers (internal and external). Oversee all aspects of UA tracking including coordinating with contracted collection agency, track results for reporting, and communicating results to team and court. Complete Relapse Prevention Plans with clients as needed following relapse.
Assures that all clinical and administrative documentation, written and online, is completed in a thorough, accurate, timely and clinically appropriate manner.Develops individualized Recovery Plans with each client, including measurable goals and objectives for recovery and wellness, with a focus on reducing recidivism. Reviews and updates Recovery Plans with each client as frequently as needed, but at a minimum every 180 days. Formulates crisis plans with clients when needed, addressing all required elements. Completes Discharge Summaries in a timely manner, as clinically appropriate. Completes administrative documentation in a timely and accurate manner, such as Time Sheets, Time Off Request forms, Mileage Reimbursement forms, etc. Provides all required annual documentation to Human Resources and complete all required Relias Learning training in a timely manner. Assures confidentiality is always maintained.
Job ScopeClinicians work semi-independently managing a caseload of clients in a highly competent and professional manner. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, clinical outcomes, and liability implications.

Communication/Customer Contacts This position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners. Must be able to successfully navigate different systems and partners, with the goal of promoting success for FFACT clients. Must also abide by all confidentiality laws and pertinent codes of conduct.
Knowledge and SkillsIn depth understanding of the SUD population, ASAM criteria, modalities of treatment and services for adults and special populations. Ability to operationalize those treatment options within the managed care environment of the Pierce County MCO/ASO. Skilled in a broad range of SUD treatment methods & techniques for working with diverse populations and various client issues. Direct clinical experience in the assessment and treatment of a wide range of SUD needs. Knowledge of and experience with a wide variety of social systems and the general public. Ability to communicate verbally and in writing, in a clear and concise way, often on complex issues. Demonstrated problem-analysis and decision-making abilities. Thorough knowledge of the American Society of Addiction Medicine (ASAM) criteria. Well-developed skills in managing time, priorities, and paperwork; excellent written and oral communication skills Must be able to work effectively in a team environment and the ability to collaborate effectively with external partners Understanding of the importance of clear, professional boundaries, and maintenance of such Cultural competency
Education and/or ExperienceA Substance Use Disorder Professional license issued by the Washington State Department of Health is required. Associate Degree in Chemical Dependency/Substance Use Disorder or Behavioral Sciences is also required. Bachelor's degree in Behavioral Sciences with 2 years of experience as a SUDP is preferred. Other combinations of education and experience that provide the required familiarity and skills may be considered.
Job ConditionsMust be able to drive a car; possess a good driving record; provide proof of auto insurance; be able to work both in and out of the office and go into clients' residences, which may require climbing stairs and walking on uneven terrain. Must be able to speak and hear at normal conversational levels. Must pass the background check in order to see program clients in the Pierce County Jail.
