We are looking to hire those with internal drive and compassion in order to promote our company's mission, vision and values through outcome-based strategies which support the principles of Recovery and Trauma informed care.Job DescriptionThis position provides direct clinical services to clients in the Felony Mental Health Court/Forensic Assertive Community Treatment Program (FMHC/FFACT). Completes mental health/eligibility assessments for referred individuals. Provides mental health treatment for FFACT clients, with the goal of reducing recidivism into the Jail, as well as when appropriate, working to reduce reliance on ER's and use of both medical and psychiatric inpatient hospitals. Collaborates with clients to develop treatment and crisis plans. Attends weekly court staffing and court hearings; completes weekly status reports for the court. Provides crisis intervention and support to FFACT clients in the community as necessary; responsible for rotating on-call duties. Responsibilities1. Provides effective clinical case management services to improve FFACT clients' ability to live successfully in the community. Performs on-going clinically appropriate assessments of clients. Develops, with consumer voice, the goals to be addressed in individual treatment plans, tailored care plans, and crisis plans, in accordance with the appropriate model of care. Provides behavioral counseling, crisis and stabilization services to clients. Maintains professional clinical boundaries with clients. Provides case management, and referral as needed. Provide advocacy with the court system, the Jail and other community supports as needed. Maintains clinical competency and resource knowledge required to meet changing consumer, community, and industry trends. Provide written reports for the court and attend scheduled court hearings with clients on a weekly basis. Attend daily staffings and weekly team meetings. Assures confidentiality is always maintained. 2.Completes clinical documentation in a timely, accurate manner. Ensures that all assessments are completed in a clinically appropriate and timely manner. Develops individualized Recovery Plans with each client, including measurable goals and objectives for recovery and wellness, with a focus on reducing recidivism. Reviews and updates Recovery Plans with each client as frequently as needed, but at a minimum every 180 days. Formulates crisis plans with clients when needed, addressing all required elements. Completes Discharge Summaries (Jail to the community and from the program to regular case management) in a timely manner, as clinically appropriate. 3.Completes progress notes on time for each service providing a clear, concise description of the client's progress towards treatment goals, being sure to document any risk concerns. Current YTD progress note compliance percentage (will be calculated for PR). Progress notes routinely meet medical necessity requirements. 4.Practices effective caseload management in ways that meets productivity expectations. Current YTD productivity (will be calculated for PR). 5.Completes administrative documentation in a timely and accurate manner. Completes and submits Time Sheets, Time-Off Requests, Mileage Reimbursement forms, etc. accurately and on time. Provides all required annual documentation to Human Resources and completes all required Relias Learning training in a timely manner. Job ScopeTherapists work semi-independently managing a caseload of consumers in a highly competent and professional manner. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, clinical outcomes, and liability implications. Communication / Customer ContactsThis position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners. Must be able to successfully navigate different systems and partners, with the goal of promoting success for FFACT clients. Must also abide by all confidentiality laws and pertinent codes of conduct. Education and/or ExperienceM.A. degree in a mental health related field plus two years of mental health experience under the direction of a Mental Health Professional and must be a recognized Mental Health Professional (MHP) as defined in WAC 246-341-0200/RCW 71.05.020. Must be eligible for a Washington State Department of Health Agency Affiliated Counselor Registration or other licensed mental health credential. Experience conducting mental health assessments required.Job Conditions Work is provided both at the office, in the community and in the jail. This position requires travel to community locations where clients may be and to the Pierce County Jail on a very frequent basis. When in the office, work is generally performed in an open cubicle environment, involving frequent interruptions. Must be able to speak and listen at normal conversational levels. The job requires the ability to move about all facilities freely, which may include going up and down stairs. This position also requires the ability to drive around the community, so a valid driver's license, insurance and a good driving record are required.
