Job SummaryOperating throughout Harris County Sheriff's Office (HCSO) jails, the Service Linkage Worker Lead (SLWL) provides support to the HIV Project Coordinator and the HIV Services department. The department provides eligibility, retention to care and service linkage to individuals diagnosed with Human Immunodeficiency Virus (HIV) that are incarcerated in HCSO jails. The department assists clients with navigation of resources and services while incarcerated and upon release from custody. SLWL collaborates with the HIV Project Coordinator by participating in departmental peer review and chart audits to ensure deliverables are in compliance and within the scope of services.
