Job SummaryEstablish a liaison relationship with patient/families, community partners, and significant others for needed services. Make continued rounds at the clinic to receive referrals from the staff. Facilitate the process for inmate care concerns through collaboration with all Correctional Health departments and the Harris County Sheriff's Office.
