Job Summary - CORRECTIONAL HEALTHThe Patient Navigator - EC Frequenters coordinates comprehensive patient care across the healthcare continuum and provides individualized support to patients as their resource and advocate. The navigator collaborates with other disciplines and departments throughout Correctional Health and ensures that patients have full, equitable and convenient access to clinical services.The navigator performs program related functions. The navigator also provides direct clinical nursing care within her/his scope of practice, education, and training; and participates in performance improvement, research processes/utilization and community activities.
